Frequently Asked Questions about Survey Participation
How much does participation cost?
There is no cost to participate.
What do we get for participating?
- Participating organizations receive an electronic copy of the final report for $100 (a $325 savings)!
What does the data collection process look like?
- Each participating organization will be provided with an Excel/CSV document to complete their data collection. Once the data collection is complete, the Excel/CSV document will be sent back to LMI for import into the survey. By utilizing the Excel/CSV document allowing LMI to import your data we hope to reduce the time spent collecting and submitting the needed information.
- Organizations can also submit their data online through the survey site, this is a manual process. Participants will be provided with login information upon request. Participants are unable to upload data.
What if the organization has multiple locations?
- If there are multiple hospitals/locations within your system you would like to include, data collection forms will need to be completed for each location. Each location is eligible for the discounted final report.

- Data collection can include as many or as few as you would like. Multiple people can have access to the online site and/or the excel forms can be shared.
